Perinatal Support Specialist
Pregnancy and postpartum can feel like a whirlwind of emotions—excitement, joy, exhaustion, and sometimes, deep uncertainty. If you’re feeling overwhelmed, unsure of where to turn, or just in need of someone who truly understands what you’re going through, you’re not alone. I’m Yahdiya Coleman, and I am here to help you navigate this journey with care, knowledge, and unwavering support.
As a Perinatal Support Specialist, my role is to ensure that Black women and birthing individuals have access to the mental health and wellness resources they need throughout the perinatal period—the time from pregnancy through one year postpartum. I help enroll participants in our Birth Network, conduct clinical assessments, match mamas with providers, and facilitate perinatal support groups where you can connect with others who truly understand.
With over seven years of experience in social work, case management, mental health services, and community outreach, I bring both expertise and deep compassion to this work. I am trained in Perinatal Mental Health through Postpartum Support International, have completed Substance Use Disorder Training, and am working toward my Doula certification to provide more hands-on support to birthing individuals. My background has taught me that access to culturally affirming care is life-changing, and my goal is to help you feel seen, heard, and fully supported.
You deserve a care team that honors your voice and respects your experience. Whether you need practical resources, mental health support, or simply someone to walk with you through this season, I am here. Your journey matters—and I’d be honored to be a part of it.
